All rising seventh grade students are required by the Florida Department of Health to provide documentation that a tetanus diphtheria-pertussis vaccine (Tdap) was administered prior to entering the 7th grade.

You may contact your health provider or St. Johns County Health Dept. at 825-5055 for assistance. Upon receiving the immunization the revised immunization record, complete with all previous immunizations must be submitted to the school.
