Parent Notice - Enrollment

In order to enroll --

Parents must provide:
Copy of Last Report Card
2 proofs of residence - bring photocopies (acceptable proofs include utility bill, mortgage or rent receipt, copy of lease; not Drivers License)
Medical records HRS form 680 obtained from Doctor or Health Dept (due by July 1st)

Student must provide:
A reflective essay on ONE of the following topics; preferably typed; approximately 300-words:

Essay Topics Choose ONE:
Why do I want to go to ABLE School?
Who am I and what do I know?
Who are the most meaningful people in my life and why?
What will be my ideal job in the future (and why)?
If I ran the world what would I change? (What would it look like?)
